Provider Ramp-Step Outcome Reconciliation + Progressive Decision Foundation
This gate prevents a completed ramp step from silently becoming more traffic. It binds the exact v0.12.44 authorization and execution to observation evidence, telemetry, receipt reconciliation, callback-conflict checks, an armed kill switch and a separate dual-control decision. Staging can prove the contract only: the 5% rehearsal ends in HOLD, the planned next step remains 15%, next-step authorization stays NO, accepted live requests stay 0 and network dispatch stays off.
Outcome self-test
READYRuns 38 server-owned checks as separate Cloudflare-safe Worker invocations. The live QA path validates the exact guard functions without rebuilding the full cumulative provider fixture chain inside one request.
