Provider Ramp Continuation Authorization + Bounded Step Execution Foundation

This gate makes the next traffic step a separate, one-time decision. It binds the exact first-request outcome decision to an independent approver, telemetry thresholds, an armed kill switch, a minimum observation window, a short expiry and the approved 0% → 5% step. Staging rehearses the contract only: accepted live requests stay 0 and network dispatch stays off.
